Release date is an estimate.
The name 99 Boxes is derived from the vector boxes inside, but doesn't appear anywhere in the intro. The closest to an onscreen title is "Another Awesome Rebels BBS-Commercial", but is also known as "BBS Is Commercial" (from the Amiga Demo Scene Online Reference Manual) and "BBS Commercial" (from Demon Download).
Speed Grove is not officially affiliated with Rebels.
